国内近10年阴茎背神经切断术治疗早泄研究的系统评价

摘要 目的：调查阴茎背神经治疗早泄的临床应用情况，不良反应及相关因素。方法：以“阴茎背神经切断术”为检索词，检索中国期刊全文数据库(CNKI)(2000年1月-2011年1月)，收集国内期刊发表的阴茎背神经切断术治疗早泄临床研究，并分析纳入研究中诊断标准、手术方法、疗效、不良反应、术后随访。结果：最终纳入19篇文献，设计均为自身前后对照观察性研究，共2210例(不包括对照组病例)患者，手术方法上大部分文献有着相同的诊断和疗效标准，有效率的疗效在65%和99.20%之间，共发生不良反应134例，不良反应类型包括阴茎头部麻木，局部疼痛、射精迟缓、伤口愈合不佳等。结论：阴茎背神经切断术治疗早泄有一定的疗效，但缺乏合理的试验设计和远期疗效及安全性的随访，还需要进一步的验证。

Systematic Review on the Application of selective resection of the dorsal penile nerves in patients with premature ejaculation in recent 10 years

Objective: to investigate the application and badness reaction of selective resection of the dorsal penile nerves in patients with premature ejaculation.Methods China NationaI Knowledge lnfrastructure(CNKI)database was searched with the key word of “selective resection of the dorsal penile nerves” from January 2001 to January 2011 to collect the literature regarding clinical study on selective resection of the dorsal penile nerves，as welI as diagnostic criteria, surgical method, efficacy, adverse reactions, postoperative follow-up were analysed.Results Finally,19 documents involving 2110 patients were included．most of the literature on operation method have the same diagnosis and curative standard,Efficient curative effect is between 65% and 99.20%.Adverse reactions occurred in 134 patients，including the numb of glans、local pain, delayed ejaculation, the infected cut.Conclusion: selective resection of the dorsal penile nerves have a certain effect,safety is good, but the lack of reasonable design of experiment and long-term efficacy and safety of follow-up, stillrequires further investigation.
