显微镜下精索内静脉高位结扎与传统Palomo术式治疗精索静脉曲张对比研究

［摘要］ 目的 对比分析显微镜下精索内静脉高位结扎与传统Palomo术式治疗精索静脉曲张的手术并发症发生率及精子质量改善情况。对象与方法 研究对象为我院收治的98例精索静脉曲张患者，其中58例接受了传统的Palomo术式，40例接受了显微镜下精索静脉结扎术，对两组手术时间、并发症发生率以及术前、术后精液分析结果进行对比分析。结果 显微镜组的手术时间长于传统Palomo组（p<0.05），显微镜组术后的阴囊水肿和附睾炎发生率均显著低于Palomo组（p<0.05），两组术后复发率差异无统计学意义，两组术后精子质量均有显著改善，但显微镜组术后精子计数、密度和或率均明显高于传统Palomo组（p<0.05）。结论 显微镜下进过静脉高位结扎术治疗精索静脉曲张并发症少，精子质量改善程度优于传统Palomo术式。合肥市第一人民医院泌尿外科刘骋

Study on high-approach microsurgical varicocelectomy and traditional Palomo operation in the treatment of varicocele

［Abstract］ Objective To evaluate the effects of high-approach microsurgical varicocelectomy and traditional Palomo operation in the treatment of varicocele.

Meterial and method 98 inpatient with varicocele were divided into two group ,one group(A group) with 40 patients received high-approach microsurgical varicocelectomy ,and another group (B group) with 58 patients received traditional Palomo operation. The operation time, complication and quality semen of pre-operation and post-operation between groups were analyzed contrastively. Result The operation time of group A longer than group B（p<0.05）, but the incidence of scrotum and epididymitis is lower significantly in group A than in group B （p<0.05）and the recurrence was not significant difference between two groups（p>0.05）. Three month after operation ,  the sperm count, sperm density and sperm motility of two group compared with pre-operation both were enhanced（p<0.05）, and the quality of group A was better than group B（p<0.05）, but There were no statistic difference between groups before operation（p>0.05）. Conclusion  Compared with traditional Palomo operation in treatment of varicocele, High-approach microsurgical varicocelectomy has the advantage of Lower incurrence of complication and higher improvement in quality of semen.
