经皮椎体成形术治疗脊柱转移瘤

摘要 目的: 探讨经皮穿刺椎弓根途径椎体成形术治疗椎体转移性肿瘤疼痛的效果。方法: 对 22 例椎体转移癌患者的28个椎体施行了经皮椎弓根途径椎体成形术, 采用视觉模拟评分法 (visual- analogue scale,VAS)对治疗前后患者疼痛状况进行评估、分级, 术后疼痛缓解程度采用 6 级评定法评定。术后24小时 、1 周、3、6、12 个月常规随访, 进行疼痛缓解程度评价。结果: 22例患者中 12例局麻药药效消失后疼痛缓解,10 例 24h 后疼痛缓解, 无疼痛加重病例。随访时间 12--24个月, 平均随访 14 个月。术后24小时优良率为77.3%,有效率为86.3%;术后 1 周优良率为 72.7%, 有效率为 86.3%;术后 3 个月优良率为 68.2%, 有效率为 86.3%;术后 6 个月优良率为 63.6%, 有效率为 81.8%;术后 1 年优良率为 54.5%, 有效率为 68.1%。结论: 经皮穿刺椎弓根途径椎体成形术对椎体转移性肿瘤疼痛具有良好的止痛效果, 能够有效改善患者的生存质量。辽宁省肿瘤医院骨软组织肿瘤科郑珂

Treatment of Spinal Metastasis by Percutaneous Vertebroplast
Zhengke 【Abstract】 Objective: To study the result and mechanism of percutaneous vertebroplasty (PVP) in treating pain of vertebral body metastatic tumors. Method: 28 vertebral bodies of 22 patients underwent percutaneous injections of surgical cement(polymethyl methacrylate, PMMA) under X- ray guidance, the degree of pain was evaluated and classified using visual pain scale and the prolo score (visual- analogue scale, VAS), All of the patients were examined at twenty four hour, one week, and three, six and twelve months after surgery to ascertain the development of the pain and possible changes.Result: Of 22 patients, 12 patients pain were anesised when anesthetic disappear, 10 patients pain were anesised 24h after operation, any cases had not crura pain.The time of follow- up was 12 to 24 months, the mean follow- up was 14 months. Fineness rate of operation after twenty four hour was 77.3%, effective rate was 86.3%; Fineness rate of operation after one week was 72.7%, effective rate was 86.3%,;fineness rate of operation after 3 months was 68.2%, effective rate was 86.3%,;fineness rate of operation after 6 months was 63.6%, effective rate was 81.8%;fineness rate of operation after one year was 54.5%, effective rate was 68.1%.Conclusion: The percutaneous vertebroplasty(PVP) provides satisfactory results to pain of vertebral body metastatic tumors and has efficiency to improve patient′s life quality.
